Mission
The Transitions Team partners with patients and their families to:
- Adjust to a chronic or incurable condition
- Relieve emotional, physical, and spiritual pain.
- Transition to improved wellbeing and a hope-filled life.
Transitions is a program of palliative care. The primary goal of this service is to provide family-centered care to patients with advanced, progressive illness such as lung disease, heart disease, diabetes, stroke, Alzheimer’s disease, and cancer. It is an approach to care that emphasizes patient goals, relief of pain and suffering and quality of life. Palliative care can be of benefit while the patient is still seeking curative therapy and not appropriate for Hospice. It will also benefit health care providers wishing expert assistance or guidance in pain and symptom management, goal setting, and facilitating conversations with patients and families.
