  • Have a respected spirituality

  • Solitude
  • Participate in their plan of care
  • Expect confidentiality
  • Be informed of the Hospice concept, admission criteria, services to be provided, and any charges associated with service provision
  • Refuse or discontinue service at any time
  • Be provided care regardless of race, religion, sex, or age
  • Receive care that is ethical and in the best interest of the patient, but is also respectful of the patient’s values and spiritual/religious preferences.

Heart of Hospice affirms the patient’s responsibilities:

  • To provide accurate information concerning illness and changes that will help toward the patient’s care.
  • To have an able and willing caregiver.
